Part A: The signalment, behavioral history, clinical exam and blood work will often narrow the cause of the seizure to idiopathic or structural. The first portion of this lecture will demonstrate what and how to extract information from the record and a simple exam to effectively know the cause, prognosis and best course of treatment. Clinical cases will be provided to best demonstrate these points. Part B: Cluster seizure patients frequently evolve into Convulsive Status Epilepticus and Nonconvulsive seizure/Status Epilepticus, with mortality rates as high as 50%. Attendees will gain an understanding and improved clinical suspicion of nonconvulsive seizure from analyzing video cases where the EEG has diagnosed nonconvulsive seizure. An algorithm will be presented for the in-hospital management of cluster seizure, status epilepticus and nonconvulsive seizure. Lastly, we will discuss targeted therapy to prevent cluster seizure (rescue therapy) and amelioration of post-ictal symptoms. Objectives: The attendee will be able to use patient and examination information to take an effective guess at the diagnosis, prognosis and best treatment plan – No MRI needed! Secondly, the attendee will be able to recognize and treat the most severe and emergent seizure presentations: Convulsive and Non-Convulsive Status Epilepticus. Thirdly, they will understand when and how to institute rescue therapy to prevent cluster seizure and ameliorate post-ictal symptoms.
